As a health care professional, I am required by federal law (HIPAA) to provide you with a statement of my office's privacy practices. Please print, read and sign a copy of this document and bring it with you to your first session. I will provide a printed copy for you to sign at our first session if you are unable to bring one with you. Therapy cannot begin without this document being signed.
